Enhanced Primary Care Referrals
Patients with Osteoarthritis, Osteoporosis, diabetes, depression, heart disease, emphysema, asthma, stroke and other chronic conditions may be eligible for medicare rebated sessions. Patients can be given either a supervised gym or home exercise program specific to their chronic care needs. Physical activity can reduce HBA1c levels in diabetes, reduce the risk of falls and improve function and mobility in elderly patients.
